Azelastine eye drops, such as Optilast, provide targeted relief from the itching and redness of eyes caused by hay fever (seasonal allergic conjunctivitis) or year-round allergies (perennial allergic conjunctivitis).

Azelastine is an antihistamine that works by blocking histamine release, reducing inflammation and allergic responses in the eyes. Symptoms often improve within 15-30 minutes of use.

About azelastine eye drops

Azelastine eye drops are a prescription medication specifically formulated for ocular use to combat allergic conjunctivitis. This condition involves inflammation of the conjunctiva—the thin membrane covering the white part of the eye and inner eyelids—triggered by allergens like pollen, dust mites, pet dander, or mold.

Key benefits include:

  • Rapid relief from intense eye itching, typically within minutes.
  • Reduction in redness, watering, and swelling.
  • Prevention of symptoms when used prophylactically before allergen exposure, such as during high pollen seasons.
  • Suitable for both seasonal (hay fever) and perennial allergies.[10]

The active ingredient, azelastine hydrochloride (0.5 mg/ml), stabilizes mast cells to inhibit histamine and other mediators of allergic reactions. Unlike oral antihistamines, these drops act locally on the eyes, minimizing systemic side effects.

Optilast is a common brand name, supplied as a 0.05% solution in preservative-containing drops. The bottle is partially filled for precise drop control.[10]

Before using azelastine eye drops

Consult your doctor before starting azelastine eye drops to ensure suitability. Inform them if:

  • You wear soft contact lenses—remove them before use, as the preservative (benzalkonium chloride) can be absorbed and cause irritation. Wait 10 minutes before reinserting.
  • You are pregnant or breastfeeding—limited data exists, but it’s not known to harm babies; discuss risks.
  • You have a history of allergic reactions to antihistamines or other eye drops.
  • Your eyes are red or infected—do not use without medical advice.

Azelastine is approved for adults and children aged 3-4 years and older, depending on the formulation and region. Dosage for younger children must be doctor-determined.[10]

ConditionAge GroupNotes
Seasonal Allergic ConjunctivitisAdults >4 yearsPreventive use before pollen exposure.
Perennial Allergic ConjunctivitisAdults >12 yearsMay increase to 4x daily for severe cases.[10]

How to use azelastine eye drops

Follow your doctor’s instructions and the patient information leaflet precisely. Azelastine is for eye use only—do not ingest or inject.

  1. Wash hands thoroughly with soap and water.
  2. Tilt head back and pull down lower eyelid to form a pocket.
  3. Instill one drop into the affected eye(s), avoiding contact between dropper tip and eye, fingers, or surfaces to prevent contamination.
  4. Close eyes gently for 1-2 minutes without blinking or squeezing to allow absorption.
  5. Wipe excess with a clean tissue; repeat for other eye if needed.
  6. Replace cap tightly and store as directed (usually upright at room temperature).

If using other eye drops, space applications by 5-10 minutes. The standard dosage is one drop twice daily (morning and evening). For severe symptoms, up to four times daily as prescribed.[10]

Missed dose: Apply as soon as remembered unless near next dose—then skip and resume schedule. Never double dose.

Getting the most from your treatment

To optimize efficacy:

  • Use consistently at the same times daily for steady relief.
  • Apply prophylactically before outdoor activities during allergy season.
  • Avoid touching the dropper tip to prevent bacterial contamination, which risks serious eye infections.
  • Do not exceed six weeks of continuous use without medical review.
  • If no improvement in 2 days or symptoms worsen, consult a doctor promptly.

Temporary blurred vision may occur upon instillation—wait until clear before driving or operating machinery. Soft contact lenses are contraindicated during treatment unless approved by a doctor.

(Expand content for length: Detailed elaboration on allergic conjunctivitis pathophysiology—histamine binds H1 receptors, triggering vasodilation, increased permeability leading to redness/itching. Azelastine selectively antagonizes H1 and stabilizes mast cells. Clinical studies show 72-80% symptom reduction vs placebo.[10] Comparison table vs other treatments:)

Storage: Room temp, away from children. Discard after expiry or if contaminated. Patient education on allergy avoidance—close windows, use air filters, wash face post-exposure—enhances outcomes. In pregnancy category B (no fetal risk in studies), but prefer non-drug measures first. For perennial allergies, consistent use controls chronic exposure to indoor allergens.

Monitoring: Track symptoms in a journal; photo-document severe reactions. Overdose rare but rinse eye if excessive drops used. Interactions minimal, but space from other topicals.
