About dexamethasone with antibacterial ear spray

The brand name for this treatment is

Otomize®

. This spray contains three key active ingredients that work together to address outer ear issues:
  • Dexamethasone: A corticosteroid that reduces inflammation by inhibiting immune responses causing swelling, redness, and pain.
  • Neomycin sulphate: A broad-spectrum antibiotic that prevents bacterial growth by disrupting protein synthesis in bacteria.
  • Acetic acid: Creates an acidic environment in the ear canal, inhibiting bacterial survival and growth.

Otomize® is prescription-only and suitable for adults and children aged over 2 years. It targets mild to moderate otitis externa, often triggered by trapped water after swimming, scratching, or cotton bud use, leading to a moist environment for bacterial proliferation.

The spray form ensures direct, even application to the external auditory meatus, acting locally with minimal systemic absorption. Each 5ml bottle typically contains dexamethasone 0.1%, neomycin 0.5%, and acetic acid 2%. Inactive ingredients include solvents, emulsifiers, and preservatives, which may rarely cause allergies—always check the label.

Otitis externa affects the outer ear canal, causing symptoms like pain, itching, discharge, and temporary hearing loss. Prompt treatment prevents complications like chronic infection or spread.

Before using Otomize® ear spray

Consult your doctor before use to ensure suitability. Inform them if:

  • You are pregnant or breastfeeding, as safety data may be limited.
  • You have a perforated eardrum or grommet in place—neomycin can cause complications like hearing loss if it reaches the middle ear.
  • You have a history of allergic reactions to medicines, especially aminoglycoside antibiotics like neomycin or preservatives.

Avoid use if you suspect a fungal infection, viral cause (e.g., herpes), or middle/inner ear involvement, as this spray targets bacterial outer ear issues. Diagnosis by a healthcare professional is essential to rule out alternatives like otitis media.

Neomycin can rarely cause contact dermatitis or sensitization with prolonged use. If you have kidney issues or are on other ototoxic drugs, discuss risks.

How to use Otomize® ear spray

Read the patient information leaflet (PIL) provided with the pack for detailed instructions.

Preparation: Shake the container gently. For first use, prime the pump by pressing down several times until a fine spray appears. Avoid directing at eyes, face, or inhaling.

Dosage: Use

one spray into the affected ear(s) three times daily

. Insert the nozzle tip into the ear canal and press once per application.
  • Continue for 2 days after symptoms resolve.
  • Do not exceed 7 days total unless advised by your doctor.

If you forget a dose: Apply as soon as remembered, then resume schedule. Do not double dose.

Storage: Keep at room temperature, away from children. Do not use after expiry.

If no improvement after 7 days, or symptoms worsen (e.g., increased pain, discharge, fever), stop use and seek medical advice—may indicate resistant infection or different cause.

Can this ear spray cause problems?

Most users tolerate Otomize® well, but side effects can occur:

  • Common (mild): Temporary burning, stinging, or irritation upon application—usually resolves quickly.
  • Rare: Rash, itching, or allergic reaction to neomycin/preservatives (e.g., contact dermatitis). Discontinue and consult a doctor.
  • Very rare: Hearing disturbances or balance issues if misused with perforated eardrum.

Overuse risks antibiotic resistance or fungal overgrowth. Monitor for worsening symptoms. Report persistent issues to your pharmacist or GP.

Frequently Asked Questions (FAQs)

What is Otomize® ear spray used for?

It treats mild-moderate outer ear infections (otitis externa), relieving inflammation, pain, itching, and infection via its three active ingredients.

How quickly does Otomize® work?

Relief often starts within 48 hours, but complete 2 days post-symptom resolution, up to 7 days max.

Is Otomize® safe for children?

Yes, for children over 2 years with a prescription; not for under 2s.

Can I use Otomize® if I have a perforated eardrum?

No—consult your doctor first, as it may cause harm.

Stop use and see your doctor for re-evaluation.

Additional Considerations for Otomize® Use

To maximize effectiveness and safety:

  • Clean the outer ear gently; avoid cotton buds which can worsen infection.
  • Keep ears dry—use swim caps or earplugs if prone to swimmer’s ear.
  • Avoid if you have viral/fungal infections or allergies to components.

For prevention, dry ears thoroughly after swimming/showering. If recurrent, investigate underlying causes like eczema or allergies.

Generic versions (dexamethasone/neomycin/acetic acid spray) are available post-Otomize® brand discontinuation, offering equivalent efficacy at similar cost (~£3.27 for 5ml).

Always follow professional advice; this is not a substitute for medical consultation.
