Joint symptoms are the hallmark of arthritis. These include a combination of pain, stiffness, warmth, redness, and swelling that persist or recur. Pain from arthritis can be constant or intermittent, occurring at rest or during movement, and may affect one or multiple body parts. Swelling often makes the skin over the joint red, warm, and tender to the touch, lasting more than three days or recurring more than three times a month signals the need for medical attention.

Warmth and redness indicate inflammation, particularly in autoimmune forms like rheumatoid arthritis (RA), where the immune system attacks the joint lining (synovium), causing thickening and excess fluid production. Difficulty moving the joint, such as limited range of motion, is common and may improve slightly with activity but worsens over time.

Stiffness

Warmth and Redness

These signs of active inflammation are more prominent in RA, where the synovium becomes inflamed, leading to red, warm joints. OA may cause mild warmth from overuse but less redness.

Swelling can be hard (bony growths like osteophytes) or soft (fluid buildup). Persistent swelling over three days or frequent episodes warrants evaluation. In fingers, knobby swellings form from bone spurs.

Arthritis affects various joints differently. OA commonly targets weight-bearing joints like knees and hips; RA starts in small hand and foot joints symmetrically. Track affected areas:

  • Fingers and Hands: Bony spurs cause swelling, tenderness, and pain at thumb base; Heberden’s and Bouchard’s nodes in OA.
  • Wrists and Elbows: Stiffness and grating sensations.
  • Knees: Grating (crepitus), buckling, swelling; pain on stairs.
  • Ankles and Feet: Big toe pain, ankle swelling.
  • Shoulders and Hips: Groin/buttock pain radiating to thigh/knee.
  • Jaw and Lower Back: TMJ involvement or spinal OA.

Symmetric involvement (both sides) suggests RA; unilateral or weight-bearing focus indicates OA.

  • Fatigue: Profound tiredness during flares.
  • Fever and Weight Loss: Low-grade fever, unintended loss.
  • Skin Changes: Rashes, rheumatoid nodules, scaly itchy skin (psoriatic arthritis).
  • Nail Changes and Hair Loss: Pitting nails or patchy loss.
  • Extra-Articular in RA: Dry eyes/mouth, lung issues, heart inflammation.

Muscle weakness, instability (e.g., knee giving way), and crepitus (grinding sounds) accompany joint issues.
